Support coordination is designed to help you effectively utilise the supports outlined in your NDIS plan. It is a capacity-building service that assists you in:
Support
Level 1: This level helps you connect with informal, community, and funded supports, enabling you to maximize your plan and pursue your goals.
Level 2: This level focuses on building your skills to understand and utilise your plan effectively. A support coordinator will work with you to ensure a mix of supports that enhance your ability to maintain relationships, manage service delivery, live independently, and engage with your community.
Level 3: This is a more intensive level of support designed for individuals with complex needs. A specialist support coordinator will help you manage challenges in your support environment and ensure consistent service delivery.
A psychosocial recovery coach is a new NDIS service aimed at assisting individuals with psychosocial disabilities in gaining greater control over their lives and effectively managing the complexities of daily living.
